Neighbourhood Model for Seniors at Risk
The Neighbourhood Model for Seniors at risk is a collaborative project involving CityHousing Hamilton, Catholic Family Services (www.cfshw.com), and St. Joseph's Home Care.

Location: 226 Rebecca Street & 360 King Street East managed by CityHousing Hamilton.

Our on-site Support Services Coordinator manages and co-ordinates the services for the Neighbourhood Model for Seniors at Risk clients.

A team of on-site Personal Support Workers who are on site Monday to Sunday 7am to 7 pm provides essential support services.

Other services provided include:
  • congregate dining
  • social and wellness promotion events
  • crisis intervention
  • information and referral
  • advocacy
Eligibility:
  • Able to direct own care
  • 60 years of age or older
  • Already living at 226 Rebecca Street & 360 King Street East or in the adjacent neighbourhood. Anyone wishing to apply for CityHousing Hamilton can do so by calling Access to Housing 905-524-2228.
