Goals:

The goal of the Supportive Housing/ Housing with Supports Service Delivery Model is to provide a cost effective, fully integrated model of tenant support, which will maximize the potential for seniors to age in place permanently while maintaining the highest level of independence possible.

Focus:

The focus of assistance in the Supportive Housing/ Housing with Supports Programs is to promote and strengthen the individual’s health and social environment. We share in the decision making process with clients and families, and search for the best possible ways to maintain independent living while respecting the dignity and personal choices of the individual.
